Reason codes

rubric-v1
First calibrated grading — the claim's first number under rubric v1 (mechanism × documentation weight).
rubric-v2
Rubric v2 migration — the headline became the product of two bars: the Miracle Meter (if the facts are true, could nature do this?) × Evidence (did it happen as reported?).
rubric-v3
Rubric v3 — refuted-but-unconfessed claims floored below merely-undocumented ones (Evidence ≤ 8), every grade names the disputed-fact kernel it scores, the Gold evidence bar rose to 80, and Disproven now distinguishes confessed from refuted.
